form below and complete it before you come to the studio for the first. PLEASE PRINT and include your email address so we
can contact you with workshop reminders and special offers from time to time.
For 2012 we are asking ALL students
to complete this form, even if you have already done so in the past to allow us to update our information as we move
towards the implementation of our online studio management program. Our goal is to allow you to track your classes and
register online for workshops, and you can help us by providing the current data we need to get it done.
